Our case management service is designed to support families in understanding their loved one's disease progression and navigating available benefits and resources. We assist with transitioning into long-term care services, ensuring that clients and families receive all the necessary support and accommodations. Additionally, we serve as a mediator to advocate for your loved one's needs when services are not being provided in a manner that aligns with your wishes. Our goal is to provide comprehensive care coordination and peace of mind for your family. 

Services include but not limited to:

*Comprehensive care coordination

* Hospital transitions

* LTC Medicaid Applications

* LTC placement to include Adult Family Homes, Assisted Living facilities, Memory Care, or Income support

* Identifying resources 

1 hour meet and great required to discuss needs of clients. A quote for services will be provided at this time depending on complexity of care needs. 

Some services maybe be reimbursed by Medicare.

We offer comprehensive assessments in Forensic, Immigration, Substance Use Disorder (SUD), and Attention-Deficit/Hyperactivity Disorder (ADHD) to meet diverse needs. 

• Forensic Assessments: These assessments are designed for legal contexts, providing insights into psychological functioning and relevant behavioral patterns to inform court proceedings or legal decisions.
• Immigration Assessments: We perform evaluations to assist individuals navigating immigration processes, focusing on mental health and psychological factors that may impact their applications or status.
• Substance Use Disorder (SUD) Assessments: Our SUD assessments evaluate patterns of substance use, mental health status, and readiness for change, helping clients identify treatment options and recovery paths.
• ADHD Assessments: We conduct thorough evaluations to diagnose ADHD, including interviews, behavioral assessments, and recommendations for interventions to support academic and personal success.

Each assessment is tailored to the individual's unique situation, ensuring accurate findings and actionable recommendations. We're dedicated to providing detailed reports to support your specific needs.

As a certified court visitor in Washington State, we provide thorough assessments and write detailed recommendations for the court to support the best interests of our client. A typical case, including case review, home visits, report writing, and court appearances, takes approximately 15 hours. In some instances, additional time may be required to complete the process. If extra hours are needed, this will discuss with you beforehand. The additional hourly rate is $200 per hour. My goal is to ensure a comprehensive and fair evaluation to assist the court in making informed decisions
